Phone keeps randomly "spitting out" my mem card?

Ok so here's the deal, my phone continuously stops reading my memory card as if the memory card weren't even in the slot. The card stores stuff no problem so I don't think it's a formatting issue but who knows with cell phones. Also, does anyone else's reset itself? Thanks.
